Health care can be difficult to navigate for children with developmental disabilities and their families. As an Early Periodic Screening, Diagnosis, and Treatment (EPSDT) service, The Bridge Program provides care coordination to help families and children access and coordinate medical, educational, social, or other services.
The Bridge Program Eligibility & Access Criteria:
Clinical Eligibility: Individuals who meet the Vermont criteria for Developmental Disabilities.
Financial Eligibility: Vermont Medicaid eligible as determined by DVHA.
Access Criteria: Individual must be under the age of 22. Children who are receiving care coordination, case management or service coordination from another AHS-funded source listed in the Bridge Program Guidelines are not eligible to receive Bridge Program Care Coordination.
