Community Care Network announces 2024 service results

Key outcomes include nearly 97,000 hours of direct service to more than 3,700 clients

 

Rutland, Vt. – Community Care Network (CCN) today announced its 2024 service results in conjunction with the release of its 2024 Impact Report.

 

“CCN is pleased to share its 2024 Impact Report, which provides an overview of many of the key service outcomes and areas of innovation seen at CCN during 2024,” said Dick Courcelle, Chief Executive Officer with Community Care Network. “Despite a number of current and upcoming changes in our system of care, CCN staff, as illustrated by this report, continue to deliver an exceptional level of service to those who seek our assistance, and we remain incredibly thankful to the community for its ongoing trust and confidence in our programs and services.”

 

Among Community Care Network’s 2024 service results and areas of advancement are:

 

  • 96,988 hours of direct service provided to 3,714 clients across the functional service areas of Developmental Services, Adult Mental Health Services, Child and Family Services, Mental Health Crisis Services and Substance Use Treatment and Recovery Services.
  • On average across all CCN-RMHS programs, 89% of client survey respondents reported that staff treated them with respect, 76% reported that they received the services they needed and 75% reported that the services they received made a difference.
  • Selection by the Vermont Department of Mental Health to become one of the first Certified Community-Based Integrated Health Centers (CCBHC) in the state, with full certification expected by July 1, 2025.  This certification and accompanying funding model will allow for improved integration of mental health, substance use treatment and recovery and primary care services, as well as increased ability to quickly custom-tailor services to identified community needs, all with an overarching objective of delivering rapid access to whole-person care.
  • Through transition to an Open Access Model, a reduction in average wait time to assessment appointment for mental health and substance use treatment and recovery services from 16 days in early 2023 to just over 1 day by late 2024. The new model offers same-day or walk-in appointments, reducing wait times and facilitating rapid access to assistance at the moment when individuals are ready to receive care.
  • Successful implementation of the Enhanced Mobile Crisis Program, aimed at providing individuals in crisis with immediate, effective, professional support when and where needed. The program has provided over 225 two-person mobile responses in the community since January 1, 2024.
  • Launch and immediate growth of the Street Outreach initiative, designed to engage with individuals in the community who are homeless, using substances or struggling to connect with supportive services. Through relationship building and a person-centered approach, the initiative has succeeded in helping individuals obtain emergency shelter or long-term housing, clothing and supplies, medical attention, and harm reduction kits, and access residential treatment, among other assistance. The program has served over 86 individuals since January 1, 2024.
  • Implementation of Medication-Assisted Treatment (MAT) for individuals in Rutland County struggling with substance use. The program, which also addresses co-occurring mental health issues, currently serves approximately 25 active MAT clients out of an overall client base of approximately 70 individuals.
  • Addition of primary care screenings to CCN-RMHS’ Substance Use Treatment & Recovery Program. Designed to review an individual’s health history and current health status, the program assesses key health indicators such as vital signs, lab results and health risk indicators to help prompt additional care coordination and referral if needed.
  • Rollout of an artificial intelligence (AI) documentation platform, Eleos, aimed at reducing documentation time and burden for clinical staff, improving the quality of clinical notes, and ultimately supporting better overall service delivery and client outcomes.
  • Successful launch of Rutland County’s first-ever Early Head Start Program, which saw 100% enrollment in its first year of operation.  The program offers high-quality, holistic care for infants and toddlers from birth to age 3.
  • Receipt of the 2024 Gold Level Governor’s Excellence in Worksite Wellness Award, presented by the Vermont Governor’s Council on Physical Fitness and Sports and Vermont Department of Labor.
