Targeted Case Management

Targeted Case Management (TCM) is a Medicaid State Plan service that provides assessment, care planning, referral and monitoring.  Services are designed to assist adults and children to gain access to needed services.

TCM eligibility/access criteria:

  1. Clinical: Individuals who meet the Vermont criteria for Developmental Disabilities.
  2. Financial: Vermont Medicaid eligible as determined by DVHA.
  3. Access Criteria: TCM is available for adults age 21 and over and children under 21 when the agency has exhausted Bridge Program funding and who are not receiving HCBS or other service coordination/case management services.
