The Bridge Program: Care Coordination for Children

The Bridge Program is an Early Periodic Screening, Diagnosis and Treatment (EPSDT) service that provides support to families in need of care coordination to help them access and/or coordinate medical, educational, social or other services for their children up to age 22 with developmental disabilities.

Bridge eligibility/access criteria:
  • Clinical: Individuals who meet the Vermont criteria for Developmental Disabilities.
  • Financial: Vermont Medicaid eligible as determined by DVHA.
  • Access Criteria: Individual must be under the age of 22.  Children who are receiving care coordination, case management or service coordination from another AHS-funded source listed in the Bridge Program Guidelines are not eligible to receive Bridge Program Care Coordination.
