Community Care Network’s Street Outreach Program, launched in late 2023 in collaboration with Turning Point Center of Rutland, Rutland County Health Partners, and the Rutland City Police-Community Resource Team, has been making significant strides in reaching and supporting vulnerable, unhoused individuals in the community through persistence, compassion and trust.
