Community Care Network is now a Certified Community-Based Integrated Health Center, (CCBHC). Under the CCBHC model, CCN is focused on offering accessible, integrated care — including mental health services, substance use disorder treatment and physical health screenings — and strives to ensure high-quality, evidence-based, and trauma-informed care.

CCN’s skilled and experienced team of care professionals are qualified to deliver a comprehensive range of supportive services to those who seek our assistance – including adults, children, and families, and regularly interfaces with a variety of primary care physicians, other providers and community partners to ensure care continuity for clients.

As a CCBHC, and in concert with a statewide focus on expanding quality healthcare for all, CCN is committed to inclusive service accessibility, meaning that no individuals will be turned away, regardless of their ability to pay, place of residence or age.

CCBHC History

In 2014, the Protecting Access to Medicare Act (H.R. 4302) passed in Congress. Section 223 of the legislation establishes a Certified Community Behavioral Health Clinics (CCBHC) program to improve mental health services in select states. It establishes clear definitions and service guidelines for CCBHCs, creating a common definition for high-quality, accessible, and comprehensive care. It also outlines two new payment models to improve reimbursements by using costs of services provided rather than fee schedules.

What is a CCBHC?

A Certified Community Behavioral Health Clinic (CCBHC) is a specially-designated care center that provides a comprehensive range of mental health and substance use services. CCBHCs serve anyone who walks through the door, regardless of their diagnosis and insurance status.

CCBHCs must meet standards for the range of services they provide and are required to get people into care quickly. The CCBHC model requires:

  • Crisis services to be available 24 hours a day, 7 days a week.
  • Comprehensive mental health services to be available so people who need care don't have to piece together the mental health support they need across multiple providers.
  • Care coordination to be provided to help people navigate mental health care, physical health care, social services, and the other systems they are involved in.

What are the benefits of a CCBHC?

CCBHCs are held to a high standard of care. They must:

  • Help people improve their health by making it easier to get into treatment.
  • Keep people from going into the hospital when they don’t need hospital care.
  • Make services available regardless of your ability to pay or where you live.
  • Offer 24/7 mobile crisis response.
  • Provide integrated, evidence-based mental health and substance use treatment services, including medication for opioid use disorder (MOUD).
  • Provide high-quality care, with timely access, quality reporting, and staffing and coordination of social services, criminal justice, and education systems.
  • Build better relationships between hospitals and community health care providers.

What services does a CCBHC provide?

CCBHCs must provide nine core services to promote physical and mental healthcare integration for anyone served within the CCBHC. CCBHC Core Services include:

  • Crisis Mental Health Services. These include 24-hour mobile crisis teams, emergency crisis intervention and crisis stabilization.
  • Outpatient Mental Health and Substance Use Services. These include therapy, medication management, and other therapeutic interventions.
  • Person- and Family-Centered Treatment Planning: This includes risk assessment and crisis planning.
  • Community-Based Mental Health Care for Veterans
  • Peer & Family Support and Counselor Services
  • Targeted Care Management
  • Outpatient Primary Care Screening and Monitoring: This includes a review of key health indicators and health risks.
  • Psychiatric Rehabilitation Services
  • Screening, Diagnosis and Risk Assessment

Am I eligible for CCBHC services?

CCBHC services are available to any individual in need of care who has a mental health and/or substance use disorder. CCBHCs will provide care regardless of ability to pay, caring for those who are underserved, have low incomes, are insured, uninsured, or on Medicaid, and those who are active-duty military or veterans.

    Community Care Network’s suite of supportive services under the CCBHC model have been strategically designed and developed with additional consideration to the health areas of greatest need within Rutland County, Vermont, as well as several adjacent towns outside of Rutland County in Vermont and Washington County in eastern New York.
